A single cane or crutch is generally used on the side away from the lower extremity with the problem and is advanced with that limb in the normal reciprocal gait pattern. When two ambulation aids are used, the most common gait patterns are the two-, three- and four-point alternating gait patterns and the tripod and swinging gait patterns.

In the two-point alternating pattern, the left cane is advanced with the right foot, the right cane with the left foot. Reciprocal motion is promoted and speed is maintained. The three-point gait is used when only one lower limb is affected. The two aids and the affected leg are advanced together, then share the load as the unaffected leg is advanced. For more stability and maneuverability in small spaces or crowds, the four-point pattern is used; the sequence is right cane, left foot, left cane, right foot. Only one of the four "points" is off the ground at any one time.
