Changing an ostomy appliance
ALL OSTOMY SYSTEMS consist of a skin barrier and a pouch; with a one-piece system, they're already connected. Some skin barriers are presized and others must be cut to fit. DO * Ensure your patient's privacy. Have him lie supine to smooth his abdomen for easier application. Wash your hands and put on clean gloves. * Empty the current pouch. Remove it and place it in an appropriate biohazard container or leave it attached to the skin barrier. * Clean the peristomal skin with warm water or special clear-rinsing cleanser. Pat it dry with a clean towel or gauze. * Observe the stoma color (which should appear reddish pink and moist) and note whether the stoma is a budlike protrusion or flush with the skin. Assess for edema, trauma, and skin breakdown. * Using the manufacturer's guide, measure the stoma and document the size. * Choose the appropriate appliance for your patient. Using the manufacturer's guide as a template, cut the opening to fit his stoma. * Check the manufacturer's recommendations for using a skin sealant. If using a skin sealant for better adhesion, apply it where you'll apply the skin barrier and let it air dry. * Have your patient immediately place his hand over the barrier so the warmth can activate the adhesive. * If the system has a drainable pouch, fold the open end up once and close and clamp it. * Remove your gloves and wash your hands.
* If the patient uses an ostomy belt, make sure it's snug and horizontal over his stoma. * Document your care and the patient's response. DON'T * Don't scrub the peristomal skin. * Don't apply lotion, which would prevent adhesion. * Don't apply the system to wet skin.
