Antibiotics: Use Only When Necessary

 

A disturbing new study was recently published in The Journal of the American Medical Association that appears to demonstrate an association between the use of antibiotics and the development of breast cancer.

It cannot be determined from this study whether antibiotics cause breast cancer or what the factors is that may be related to the development of breast cancer. "Although further studies are needed," the authors conclude, "these findings reinforce the need for prudent long-term use of antibiotics."

This recommendation resonates with strong statements from the Centers for Disease Control with regard to the development of antibiotic resistance. When antibiotics are not used in the correct situations, are overused, or are not used as directed, resistance can develop.

What this means is that the bacteria that are supposed to die, when exposed to appropriate antibiotics, do not. They in effect become "stronger" and more difficult to kill and as such pose a problem if and when others are infected. Scientists then need to develop new antibiotics. The new antibiotics will be more costly, and if they are used inappropriately, will ultimately become still less effective.

Facts About Antibiotic Resistance
The following are some facts about antibiotic resistance from the Centers for Disease Control:

· Antibiotic resistance has been called one of the world's most pressing public health problems.

· The number of bacteria resistant to antibiotics has increased in the last decade. Nearly all-significant bacterial infections in the world are becoming resistant to the most commonly prescribed antibiotic treatments.

· Every time a person takes antibiotics, sensitive bacteria are killed, but resistant germs may be left to grow and multiply. Repeated and improper uses of antibiotics are primary causes of the increase in drug-resistant bacteria.

· Misuse of antibiotics jeopardizes the usefulness of essential drugs. Decreasing inappropriate antibiotic use is the best way to control resistance.

· Children are of particular concern because they have the highest rates of antibiotic use. They also have the highest rate of infections caused by antibiotic-resistant bacteria.

· Parent pressure makes a difference. For pediatric care, a recent study showed that doctors prescribe antibiotics 65% of the time if they perceive parents expect them; and 12% of the time if they feel parents do not expect them.

· Antibiotic resistance can cause significant danger and suffering for people who have common infections that once were easily treatable with antibiotics. When antibiotics fail to work, the consequences are longer-lasting illnesses; more doctors' visits or extended hospital stays; and the need for more expensive and toxic medications. Some resistant infections can cause death.

When are Antibiotics Appropriate?
In general, antibiotics are appropriate to use when there is real evidence to support that an infection that is caused by bacteria. Several examples include:

· Strep throat that has been confirmed by a strep screen or throat culture.

· A urinary tract infection confirmed by the presence of white blood cells and other commonly accepted markers.

· Pneumonia that is confirmed by a chest X-ray and or confirmation of bacteria grown from the blood or sputum.

· An ear infection when the middle ear is truly infected (do not use antibiotics just because fluid is present).

· An apparent infection in the skin, especially if there is swelling and the appearance of pus.

· A sinus infection when confirmed by x-ray, CT, or significant and localized facial pain along with fever and other signs.

· Evidence of an overwhelming infection requiring hospitalization.

As physicians, we are trained to help people get better as soon as possible and have a wide variety of choices that help us to do so. Antibiotics are one of the most powerful tools available. The challenge is to use them only when needed so as to preserve their ability to do what they were designed to do. Hopefully, this new study on breast cancer risk will raise an appropriate level of caution that will help us all to exercise discretion.  Medical Supply
