Q:  Fewer and fewer doctor's offices and urgent care units are using "Ear Thermometers." When I asked why, they say they are not reliable. Do you have any information in this regard? I bought two to be used on my grandchildren and now I feel guilty.

A:  Ear thermometers grew rapidly in popularity because of their simplicity of use, rapid response (within seconds), and minimal discomfort to the patient. What a long way from the era in which I grew up and had to hold a hard glass thermometer under my tongue for what seemed like forever! Those old mercury thermometers were also difficult to read. Hopefully, you didn't drop one on the floor and have to chase little mercury bubbles all over (I speak from personal experience here, too).

The ear thermometers work by detecting infrared radiation from the ear drum. Although they are convenient, they lost favor with nurses who began noting discrepancies in the results, depending on technique, or amount of ear wax, etc. A study was done in Norway comparing rectal and ear temperature measurements on 65 patients. The ear thermometers detected fever 70% of the time in the hands of nurses specially trained to use them, compared to finding 54% of fevers when used by untrained nurses. It appears that their accuracy is questionable, and training did not make a big difference.

They are no longer used at Froedtert Memorial Lutheran Hospital for inpatients and are rarely used in the clinics. Our nurses prefer using Temp-a-dots, which are paper strips placed in the mouth for oral temperatures.

I don't think you need to feel guilty about using ear thermometers, but be sure you keep in mind that they may not be totally accurate. Thermometers, Digital Thermomters.
